Consent Forms for Sports / Music

• If you think your child might be interested in participating in any sports fixtures for GMS this academic year, please complete this link by Monday 9^th September.

• If you think your child might be interested in participating in any music trips with GMS this academic year, please complete this link by Monday 9^th September.

Parent/Carers only need to complete the links above if you think your child might like to participate in sports fixtures and / or music trips – this will act as your consent should they take part in any. Details about any sports fixtures / music trips will be given to the students and / or you nearer to any events.

You must inform the relevant staff if any of the details you have given here change.
